Medication Refill Request Form

Current clients may use this form to refill their pet’s medication. An email will be sent to our office. A team member will reply to the contact information within 24 business hours (Monday-Thursday). Alternatively, you may call, text, or email your request. Mail orders processed before 10am will ship the same day, otherwise it will ship the next morning. Most orders are delivered within 5-7 business days.

Curbside orders will be available for pick up in our entryway vestibule during regular business hours. We have an after-hours lock box available upon request. If you need to pick up after hours, call or text us and we will give you the instructions.

***By federal law, we cannot refill a prescription if we have not examined your pet in the past year***

This form will disappear when successfully submitted and an email confirmation will be sent to your provided email address (if one was provided).
